With a focus on palliative care for your non-cancer patients, attendees will participate in rotating small virtual table case study discussions with specialists from cardiology, nephrology, respirology, and hepatology to:
– Enhance medical management for seriously ill patients with non-cancer illness
– Learn how to reduce the use of non-beneficial medical care near death and support the patient and family as they transition
– Learn about a new CHF Transitions in Care Project and provide your input regarding priorities
